Been married since October, but I don't know how to go about getting my last name changed. Where do I go? Who do I call? What paperwork do I need? I don't have any idea where to start. Any help would be great!How do you get your name changed after marriage?
This is what I had to do, I live in NY:
1. Go to SS office with marriage certifcate %26amp; change your name. I dowloaded the form http://www.socialsecurity.gov/online/ss-鈥?/a> from their site to save time because it was a wait.
2. Go to DMV with marriage certificate, new SS card, and find your forms online for your state's DMV ahead of time. You'll need one to change your name and one to change your name on the title %26amp; registration.
3. Contact your car insurance; mine needed proof that my husband is a licensed driver. I had to fax over his license and proof that he carries his own insurance. It was also cheaper for me to add him to my policy than it was to be the sole insured person.
4. If you are employed, you'll have to fill out a form there too for payroll and tax purposes. You will need your husbands DOB %26amp; SS#.

You can also just change your name ';socially'; without changing it legally so you don't have to go through the hassle of changing your SSN, drivers license, credit cards, passport, bank accounts, health insurance, 401k plans, work records, etc, etc
People can just call you Mrs X when it comes to what people call you at work, what they call you when your kids are in school, etc without legally changing your name.
it's a pain. Take your marriage cert., passport, DL, SS card ...anything with your name on it and take it to the SS place. they will send you a new SS card with your name changed. Go to the DL place with your marriage cert. and they will change your name there too. Now, call every credit card, your job, mortgage, loan and send them a copy of your marriage cert....it goes on and on.
Take your marriage certificate to the social security office. They change it. You will get a new card with your new last name..then take it and the marriage certificate to the driver's license place. And just call around and let every one know your new name. You will get the new SS card in a week or two.
Contact the Social Security Office and the DMV where you live to have the name change entered and a new Photo ID with the DMV...
You will need to provide a certified copy of your signed Marriage License as evidence of legal name change and.... You're done!

Just send your marriage certificate to your local social security office and they will issue you a new social security card with your married name on it. Then you simply let everyone know that your name has changed.

Drivers' license, ID, etc... you may have to take in your marriage license...

Go to the courthouse with your marriage certificate...depending on what you're trying to change...your DL would be the DMV but usually you have to change your SSN first before they'll change your DL.
Your marriage license/certificate is enough for you to get your name changed legally
Go to your local Social Security office with your marriage certificate.
